  AZINPHOS-METHYL, DISULFOTON,  FENAMIPHOS, FONOPHOS, ISOFENPHOS
  METHAMTOOPHOS, METHYL PARATHION, PARATHION (ETHYL), PHORATE,
  PHOSPHAMTOON, TERBUFOS

  The organophosphate family of pesticides, readily absorbed by inhalation, ingestion and skin
  penetration,  affects the nervous system through cholinesterase inhibition.  Certain  of these
  compounds,  included in the Pesticide Reference Section of this Guide, are also listed above
  because of their high toxicity. Prompt treatment of exposures to these chemicals is essential to
  save lives.

  Other, less toxic  organophosphates  are listed in the Pesticide Reference Guide (Acephate,
  Bensulide, Chlorpyrifos, Diazinon, Dimethoate, Malathion, Methyl Demeton, Naled, Phosmet,
  Temephos).  These also cause nerve system damage, but fatal doses are significantly higher.

     INITIAL MEDICAL MANAGEMENT OF ORGANOPHOSPHATE EXPOSURE
               (By Cook County Hospital/Division of Occupational Medicine)

  Signs and Symptoms - Variable, but include salivation, urination, lacrimation, bronchospasm,
  and miosis.  Central  nervous system reactions may range from  anxiety, and restlessness to
  slurred speech, convulsions and coma.
  Onset of Symptoms - May range from minutes to hours.
  Immediate Intervention requires  strict attention to the A B C's
    A - Airway: May necessitate supplemental oxygen as well as frequent suctioning of secretions
    to maintain patency
    B - Breathing: May need to be assisted with a bag-valve mask or endotracheal intubation.
    Every effort should be made to correct hypoxia prior to Atropine administration.
    C - Circulation: May be compromised by dysrhythmias which may  need electrical pacing.
  Venous Access and Cardiac Monitoring - Indicated for all patients
  Decontamination - Staff must be gloved and gowned.  Patient's clothing should be removed and
  entire patient washed with "green" soap and rinsed thoroughly with water. For oral ingestion
  within 4-6 hours, employ gastric lavage followed by  activated charcoal and cathartics.
  Blood Samples for Cholinesterase Levels - must be drawn in lOcc heparinized blood tubes.
  Antidotes - Give Atropine to all symptomatic patients.
              Dose: adults 2-4 mg IV, children 0.015-0.05 mg/kg IV
              titrated to drying of secretions and resolution of bronchospasm
          Follow with Pralidoxime after adequate atropinization.
            Dose: adults 1-2 g IV, children 25-50 mg/kg over 3-5 minutes
  Drugs to avoid: Parasympathomimetics  (e.g. physostigmine,  succinylcholine),
          phenothiazines, antihistamines, opiates, theophylline, and aminophylline.

                                       PREFACE

In August 1992, the United States Environmental Protection Agency (U.S. EPA) issued new,
comprehensive regulations (Worker Protection Standard,  40 CFR Part  170) governing the
protection of employees from occupational exposure to pesticides involved in the commercial
production of agricultural plants on  farms and in forests, nurseries,  and greenhouses. The
revised regulations are intended to substantially reduce the risks of pesticide poisonings and
related injuries among agricultural workers and pesticide handlers through appropriate exposure
reduction and mitigation measures. The impetus for this project was this recently issued Worker
Protection Standard.

Anticipating an  increased visitation of medical clinics and hospitals by  employees possibly
exposed by pesticides, and an elevated need by health care providers  to obtain more information
relative to these exposures in order to better manage and treat patients, the staff in the Pesticides
Section of U.S. EPA Region 5 began in earnest to draft this Guide. The  Guide should prove
to be a valuable resource on pesticide usage, pesticide poisonings (recognition and treatment),
and appropriate  contacts for  more information on pesticides throughout U.S. EPA Region 5
(jurisdiction over the States of Illinois, Indiana, Michigan,  Minnesota,  Ohio, and Wisconsin).
Besides medical personnel at State health departments, county health departments,  poison
centers, and rural  clinics to whom this Guide is primarily  directed, the information provided
should  be useful as  well to State departments of agriculture representatives involved with
pesticides, Cooperative Extension Service  agents,  agricultural  employers,  and  chemical
companies.

This Guide was envisioned to be a resource directory to provide essential information to health
care professionals  who may  provide  services to patients (agricultural workers and pesticide
handlers)  who may have a pesticide related injury or experience adverse effects from pesticide
exposure. The information herein included was assembled  from numerous sources, including
direct contacts with many health care providers, State pesticide usage reports, State telephone
directories, information supplied from  U.S. EPA State Project Officers, and  several other
publications listed in the Bibliography at the end of this Guide.
In the compilation of this Guide, every attempt was made  to contact all State agencies, State
Cooperative Extension Services, hospitals, State and County health departments, poison control
centers, chemical manufacturers, and  others for pertinent information,  mailing addresses, and
telephone numbers. This Guide is intended, as well, to serve as an adjunct to U.S.  EPA's
Recognition and  Management of  Pesticide Poisonings,   which is an excellent source  of
information on the symptomology, hazards,  toxicity, and  treatments associated with a wide
variety of pesticide classes. This resource Guide was  modeled, in part, after a Pesticide Health
Care guide formerly developed by Glenn Shannon of the U.S. Environmental Protection Agency,
Region 10, and published by the Region 10 Pesticides and Toxic Substances Branch in 1989.

The Pesticide Poisoning Action Guide is divisible into three Sections, each  containing different
types of information pertaining to pesticide exposures and printed on differently colored paper
to facilitate reference and accessibility of the information. The first Section is a quick reference

guide for people dealing with suspected pesticide poisonings. It contains a general introduction
to the pesticide labels, and decontamination and treatments for a variety of different exposures.
The second Section is a comprehensive directory of the appropriate contacts for  pesticide
information within each State. Included is a list of the State Lead Agencies that are charged with
enforcement of The Federal Insecticide, Fungicide, and Rodenticide Act through cooperative
agreements, State and County health departments, local health clinics, county extension offices,
and poison control centers. The third Section lists the pesticides commonly used in Region 5
States by crops and otherwise, and detailed information on their acute toxicity and symptoms of
exposure by chemical class. The pesticides are cross-referenced by common name and trade
name, and addresses are provided of the main manufacturers, to facilitate usage of these lists and
for the extraction of further information, respectively.

The information in this book is by no means meant to be a substitute for professional help in
pesticide exposures  or poisonings. The mention of trade names are for reference only, and in
no way imply an endorsement for a particular pesticidal product by the U.S. Environmental
Protection Agency.

                         FOR SUSPECTED PESTICIDE POISONINGS
USING THE LABEL
One of the first things that should be done in the event of a suspected pesticide poisoning is to find and read the
label on,  or from, the pesticide container. Labels will vary by product, but all labels are required by federal
law to contain certain pieces of information that can be very useful.  The demonstration label that follows this
page illustrates information that may be available.  Notes in bold caps in the left-hand column highlight the
important areas.

       Trade Name:  The name, brand name or trademark under which the product is sold must appear on the
       front panel of the label. The brand or trade name is the one used in advertising. It does not usually
       give an indication of what active (pesticide) ingredient the product contains and is not generally the best
       means of identifying a pesticide in case of poisoning.

       Active Ingredients:  The label of each pesticide product must bear a statement which contains the
       chemical and/or common name, the percent by weight of each active ingredient and percentage by
       weight of all "inert" ingredients. If a label is available, the active ingredient information is a sure
       means of identifying the pesticide.

       Signal Word:  A key word (or words)  that gives immediate information as to the relative risk
       associated with exposure to die entire pesticide product, including all active ingredient(s) and "inert(s)".
       The signal words, in descending order of risk, are:
                               DANGER-POISON
                               DANGER
                               WARNING
                               CAUTION

       Statement of Practical Treatment: These are required for most pesticide products.  For those with a
       DANGER-POISON designation (see the demonstration label following) these statements must include
       information on hazards, symptoms,  first aid suggestions and recommended medical treatment for
       suspected poisonings. This extensive information may be provided for lower risk pesticide products,
       but it is not required.  In general, as risk diminishes, less information is required in this portion of the
       label.

       EPA Registration No.: Pesticide product labels must contain an EPA registration number.  This
       number indicates that this pesticide labeling information has been approved by the federal government.

       The rest of the label contains additional useful information:

               The name and address of the producer or registrant of the pesticide product.

               The phrase "Restricted Use Pesticide" (RUP) on the label indicates that this pesticide can only
               be applied by certified applicators because of risks to human health or to the environment that
               may result from its use.

               Details on hazards can be found in the section on Precautionary Statements.  This section also
               contains information on what protective clothing is required when applying the pesticide.

               Under the Agricultural Use Requirements the "Restricted-Entry Interval" is given; the amount
               of time that must pass before anyone can enter treated  fields, as required by the Worker
               Protection Standard regulations.

            STATEMENT OF PRACTICAL TREATMENT
Call a doctor (physician), clinic, or hospital immediately. Explain that the victim has been exposed to
galactothion and describe his/her condition. After first aid is given take victim to clinic or hospital.  If
breathing has stopped, start artificial respiration immediately and maintain until doctor sees victim.  If
swallowed - Drink 1 or 2 glasses of water and induce vomiting by touching back of throat with finger.
Do not induct vomiting or give anything by mouth to an unconscious person. Get medical attention.
In case of contact, immediately flush the skin with plenty of water while removing contaminated
clothing and shoes. See doctor immediately.  Galactothion is an organophosphate pesticide that
inhibits cholinesterase.

                        NOTE TO PHYSICIAN
Antidote - administer atrophine di-sulfate in large doses.  TWO to FOUR mg. intravenously or
intramuscularly as soon as cyanosis is overcome.  Repeat at 5 to 10 minute intervals until signs of
atrophimzation appear.  2-PAM chloride is also antidotal and may be administered  in conjunction with
atropme.  DO NOT GIVE MORPHINE OR TRANQUILIZERS. Galactothion is a  strong
cholinesterase inhibitor affecting the central and penpheral nervous system and producing cardiac and
respiratory depression. At first sign of pulmonary edema, the patient should be given supplemental
oxygen and treated symptomatically.  Continued absorption of the poison may occur and fatal relapses
have been reported after initial improvement. VERY CLOSE SUPERVISION OF THE PATIENT IS
INDICATED FOR AT LEAST 48 HOURS.

Discard clothing and other absorbent matenals
that have been drenched or heavily contaminated
with this product's concentrate.  Do not reuse
them. Follow manufacturer's instructions for
cleaning and maintaining PPE. If no such
instructions for washables. use detergent and hot
water. Keep and wash PPE separately from
other laundry.

When handlers use closed systems, enclosed  cabs,
or aircraft in a manner that meets the
requirements listed in the Worker Protection
Standard (WPS) for agricultural pesticides (40
CFR 17D.240(d)(4.6)J. the handler PPE
requirements may be reduced or modified as
specified in the WPS.
    User Safety Recommendations
Users should:
• Wash hands before eating, drinking, chewing
  gum, using tobacco, or using the toilet.
• Remove clothing immediately if pesticide gets
  inside. Then wash thoroughly and put on
  clean clothing.
• Remove PPE immediately after handling this
  product. Wash the outside of the gloves
  before removing. As soon as possible, wash
  thoroughly and change into clean clothing.
     ENVIRONMENTAL
           HAZARDS

This pesticide is highly toxic to aquatic
invertebrates and wildlife. Birds in treated areas
may be killed. Shrimp and other aquatic
organisms may be killed at recommended
application rates.  Do not contaminate water by
cleaning of equipment or disposal of wastes.


       PHYSICAL AND
  CHEMICAL HAZARDS

Do not use or store near heat or open flame.
Not for use or storage in or around the home.

    DIRECTIONS FOR USE

It is a violation of Federal law to use this
product in a manner inconsistent with its
labeling. Do not apply this product in a way that
will contact workers or other persons, either
directly or through drift. Only protected
handlers may be in the area during application.
For any requirements specific to your State or
Tribe, consult the agency responsible for
pesticide regulation.
      AGRICULTURAL USE
         REQUIREMENTS

Use this product only in accordance with its
labeling and with the Worker Protection
Standard. 40 CFR pan 170.  This Standard
contains requirements for the protection of
agricultural workers on farms, forests, nurseries,
and greenhouses, and handlers of agricultural
pesticides.  It contains requirements for training,
decontamination, notification, and emergency
assistance.  It also contains specific instructions
and exceptions pertaining to the statements on
this label about personal protective equipment
(PPE), notification-to-workers, and restricted-
entry intervals. The requirements in this box
only apply to uses of this product that are
covered by the Worker Protection Standard.

Do not enter or allow worker entry into treated
areas during the restricted-entry interval (RED
of 48 hours. The REI is 72 hours in outdoor
areas where the avenge annual rainfall is less
than 25 inches a year.

PPE required for early entry to treated areas
that is permitted under the Worker Protection
Standard and that involves contact with anything
that has been treated, such as plants, soil, or
water, is:
-coveralls over long-sleeved shin & long pants
-chemical-resistant gloves such as barrier
  laminate or vitron
-chemical-resistant footwear plus socks
-protective eyewear
-chemical-resistant headgear

Notify workers of the application by warning
them orally and by posting warning signs at
entrances to treated areas.
  STORAGE AND  DISPOSAL

PROHIBITIONS: Do not contaminate water,
food, or feed by storage or disposal. Do not
store under conditions which might adversely
affect the container or its ability to function
properly.

STORAGE: Do not store below temperature of
OF.

CONTAINER DISPOSAL: Triple rinse (or
equivalent). Then offer for recycling or
reconditioning, or puncture and dispose of in a
sanitary landfill, or by other procedure approved
by state and local authorities.

GENERAL FIRST AID AND DECONTAMINATION PROCEDURES

               CONTACT YOUR POISON CONTROL CENTER FOR ASSISTANCE

               Call for medical help immediately. If possible, obtain the trade name of the pesticide product,
               or, better yet, the pesticide active ingredients involved in the episode. Try to obtain the
               pesticide container and refer to the label for information on active ingredients and risk. Look
               for suggestions for first aid procedures and medical treatment.

               Depending on the type of exposure, use the following procedures to mitigate exposure
               effects while waiting for medical personnel:

               Eve exposure:  Hold the eyes open and wash with a gentle stream of clean running water for
               10-15 minutes.

               Skin exposure:  Remove contaminated clothing but DO NOT DISCARD.  Wash affected areas
               with soap and water. Rinse with clean running water for at least 30 minutes.  Cover area with
               a clean, soft cloth. Handle clothing appropriately to avoid additional exposures.

               Inhalation exposure:  Move person exposed away from the source of exposure to fresh air, if
               this can be done without endangering others. Start artificial respiration if breathing has
               stopped.

               Swallowed poison: Follow instructions on the pesticide label.  ONLY if recommended on
               the label, induce vomiting by  gently tickling the back of the victim's throat or have the person
               drink one or more glasses of warm, soapy water. Never induce vomiting if the individual is
               not conscious.  Give the victim lots of water to drink.  Keep the victim in a prone position,
               warm and quiet, until medical  personnel arrive.

               Bring the pesticide container with its label if you are transporting the victim to get medical
               attention.

WHEN A LABEL CANNOT BE FOUND

               If the trade name is known, use the references in Section 3.3 of this document to identify
               active ingredients contained in some common pesticide products.

               If the trade name is not known, contact the applicator or the  farmer who owns the field where
               the exposure occurred.  Commercial pesticide applicators are required by federal law to keep
               records of product, locations, and times of application for all RUP pesticides that they apply.
               Similarly, farmers are required to keep records  of applications of any RUP products they apply
               to their own fields.

               If no other information is available except the identity of the crop, growing or planned, where
               the exposure took place, use Section 3.4 of this guide to identify the pesticides that are
               commonly used on that crop.

       This section contains state-specific emergency contacts, as well as contacts for
       obtaining health information, and reporting pesticide misuse, including the following:

       Poison Control Centers will answer questions on symptoms, effects of specific
       pesticides, treatment or antidote information, and recommended laboratory tests.
       These centers are usually associated with hospitals and are in operation 24 hours a
       day. These are the best source for quick response information.

 questions about a particular pesticide use or the crops or target pest that it is registered to control.

 In Illinois, the subject matter specialists are stationed in the Illinois Cooperative Extension offices.  Here, they
 research and create their programs, and then present the programs requested by the Unit Leaders to the public.
 Also, Horticulture and IPM educators advise commercial people in their program areas.  The Unit offices
 handle most of the contact with the public.  Unit Leaders determine programming needs by surveying the

Section 3.1 includes the chemical class, pesticide type, and acute toxicity information,
specifically the oral LD50, and relative toxicity.  The oral LD50 is a standard toxicological
term which indicates the number of milligrams (mg) of pesticide per kilogram of test-animal
body weight that will kill 50% of a test animal population. Values less than 10 indicate
extremely high toxicity to mammals.  The LD50's shown apply to technical grade pesticides.
They are taken primarily from the  1993 Farm Chemicals Handbook and in most cases are
values for male rats.  The relative toxicity ratings correspond to the technical grade oral
LD50's unless otherwise indicated in the endnotes.  The ratings are defined in the table
below.  The signal word identifying the relative  toxicity of the contents is prominently
displayed on all pesticide containers.

It must be emphasized that low toxicity does not mean no toxicity, and even pesticide
products in this class that are virtually non-toxic may be irritating to skin,  eyes, and/or
mucous membranes.

Pesticide products may be encountered at full strength or diluted, in combination with
carriers and other "inert" ingredients.  These other ingredients may themselves be toxic, and

The information in this section is taken primarily from Recognition and Management of Pesticide
Poisonings and Basic Guide to Pesticides.  The Recognition book is recommended as a companion to
this guide.  It has information on toxicological effects, more detailed acute and chronic symptoms of
exposure, and treatments and antidotes for many of the pesticides that are included in this Region 5
guide.  Another resource of product information is the "Farm Chemicals Handbook," published
annually by Meister Publishing Company, Willoughby, OH.  Additional information is available from
"Material Safety Data Sheets" (MSDS), that are available from a pesticide dealer or can be obtained by
contacting the product manufacturer.  The symptom information listed here is primarily by chemical
class, except for the  "miscellaneous" pesticides at the end of this section.  Symptom information is
generally class-specific, however, the degree of the toxicity varies widely within the class. Use the
LDso's in Section 3.1, or consult the attached references for more pesticide-specific symptom
information.
