Because of the demand for this service, it is important to schedule an appointment you will be able to keep. When you make your appointment, Health Unit personnel will give you instructions for preparing for the exam. Immunizations for Foreign Travel Contact the Health Unit as soon as you know you're going overseas on official business. Some shots have to be given 60 days ahead; all have to be given at least 30 days ahead. No information about an employee's use of the program may be released without prior written consent from the employee involved. Client records are protected by Federal laws and regulations of the Privacy Act which safeguard the rights of employees to privacy. An instructor is on duty during operating hours, and records of activities are maintained. Equipment: Universal set, free weights, stationary bicycles, inclined benches, treadmills, rowing machine, showers, and sauna Hours: 6:30 a.m. to 6:30 p.m., Monday through Friday Availability: All EPA Headquarters employees Restrictions: Persons who wish to use the Stress Lab must submit a form completed by their physician stating that they have received a physical within the past 2 years and approving their participation in a fitness program. Forms are available at the Stress Lab and Health Unit. Fees: None Aerobics Classes Held every day at 11:00 and 1:00. Slimnastics Classes Held every day at 5:00. Cyclists and Joggers May use the Stress Lab for showers (but must first pass physical exam as explained above). Hands-on physical 2 weeks after the above listed workup, at which time the physician reviews the tests and all pertinent information, and consults and advises the patient as appropriate. Fees: None OSHE Tests Given: Same as above, with the exception of additional blood work, spirometry, and possible chest x-rays. ------- Availability: A physician is available twice a week at the O'Neill and four days a week at the JFK Building. Physicals are available to all employees. In September, a signup sheet is circulated to employees; depending on how many slots are available, appointments are set up with the individual employee. Prenatal Monitoring If private physician requires, Health Unit personnel can monitor monitor blood sugar and blood pressure on a regular basis. Postoperative Surveillance and Monitoring Health Unit personnel can carry out monitoring prescribed by the employee's private physician. Sick Call Employees who become ill or are injured during work may report to the Health Unit for help. Bed Rest Area Three beds are available for resting. There is a limit of 1 hour for any rest period. Employees may utilize these rest areas for the following reasons: • An employee who is not feeling well can rest up to 1 hour so that the Health Unit staff can determine if the employee is ------- capable of going back to work, should go home, or needs medical attention. • An employee who has been out of work with a serious illness and may only return to limited work (according to the private physician) can rest for 1/2 to 1 hour per day; and • Pregnant women whose private physicians so require can get off their feet for up to 1 hour per day. Environmental Control (for Agency and managerial purposes) Worksite environment surveillance: Ambient air quality, closed building syndrome, evaluation of environmental stressors, and asbestos monitoring. Hazard assessment control: Initial consultation, qualitative diagnostics, and quantitative diagnostics. Surveillance systems for high-risk management. Safety and health: Accident prevention, workmen's compensation review, comprehensive industrial hygiene audits, hazardous materials communication protocols, NIOSH/OSHA reporting and compliance, loss time analysis, and industrial hygiene protocols. ------- Counseling J.F.K. The primary function is to provide advice to Federal managers on occupational health and to provide general advice and consultation to managers and employees. Consultative assistance provided to managers consists of: • Planning evaluation/operation of comprehensive occupational health program • Safe worksite planning • Surveillance of worksite hazards • Response to hazards and surveillance information • Promotion/prevention-oriented clinical assessment and intervention services • Education/training/programs directed toward behavioral modification/ health promotion/disease prevention • Occupational health administrative, analytic, and liaison services • Respond to urgent need for care/crisis intervention • Rehabilitative or followup care fitness for duty ------- Confidentiality is the cornerstone of the counseling program. Fees: None The Health Unit does not have an Employee Assistance Program in Region 1; it is run independently by Judy Hubert at the Thomas P. O'Neill Building. Ms. Hubert can be reached at (617) 565-6533 or 6534. Fitness The J.F.K. Fitness Center opened the first week of January 1989 in the garage level of the J.F.K. Federal Building. The Fitness Center is for employees of the J.F.K. Building only. The EPA contact person for the Fitness Center is Susan Zoino; she can be reached at FTS 835-3390. The Fitness Center phone number is 617-565-1493. EPA employees should contact Susan Zoino for necessary medical and application forms which are required by the Fitness Center in order to attain membership. The Fitness Center costs EPA $60 per employee, whether the employee joins it or not. The more employees that take advantage of the Fitness Center, the more cost efficient it will be for EPA. The individual employee is required to pay a $30 annual fee upon being accepted for membership. 10 ------- The Fitness Center is open from 7a.m. to7p.m., Monday through Friday. The Center is closed on holidays and weekends. There is an exercise specialist available at all times to monitor exercise activity and to answer members' questions.The Fitness Center requires a medical clearance by the employee's own physician. Following a medical clearance, the employee will go for an assessment by the Fitness Center. This assessment includes testing: • Cardiovascular strength; • Blood pressure and heart rate; • Skin composition for body fat; • Muscular endurance; • Muscular strength; and • Flexibility. The exercise specialists will set up a workout program for each employee, and the employee keeps a workout record for weights and aerobic activity. The Fitness Center has Heart Racer (Wind Racer) bikes, Bodyguard treadmills, and weight training with Nautilus and Universal machines. There is a men's locker room with showers and women's locker room with showers. These are used by both members of the Fitness Center and by nonmembers who wish to exercise outside. Future plans of the Fitness Center include competitions between agencies, group weight loss, running clubs, and brown-bag lunch seminars on back problems, stress, etc. The Fitness Center also promotes overall wellness, corporate fitness, reducing hypertension and cholesterol, and quitting smoking. ------- Other Services Weight control groups are available, but are provided by private organizations which change hands periodically. Information on clubs or groups of this nature is posted on bulletin boards and lavatory facilities throughout the buildings. Walking groups, led by EPA employees, meet at preset locations during lunch hours. Availability: Workshops last for one hour and are held at 26 Federal Plaza. 13 ------- Other Programs The U.S. Public Health Service, the U.S. Department of Labor, and the Adelphi University School of Social Work present various conferences, such as Drugs in the Workplace, addressing the following issues: drugs: the problem, the cost to business, the solution, legal issues, drug testing and illegal drug use, how to identify and rehabilitate the drug-abusing employee. Availability: All Federal employees at any grade level who work at the 841 Chestnut building. The Health Unit does have one restriction: Only 15 percent of the Agency's population is allowed each year so that everyone has an equal chance. Employees receive an examination every 3 years. Appointments: Flyers are sent to a contact person within the personnel section. The employee has to complete a Health Risk Appraisal Form to be placed on a waiting list for an appointment. Fees: None Screenings Tests Given: Skin cancer, blood sugar, cholesterol, breast cancer. Availability: Available to all Federal employees in the 841 Chestnut Building. Appointments: Information about the screenings is sent to the employees via the personnel section. The employees go to the Health Unit to make an appointment for the screening. 15 ------- Health Education Programs Subjects: Lectures and literature are provided on a wide range of topics including headaches, diabetes, skin cancer, and high blood pressure. EPA has also sponsored health programs such as smoking cessation. Availability: The lectures, films, literature, and Health Fair are provided by the Health Unit and available to all Federal employees. Tetanus Shots Available upon request. 16 ------- Counseling Oakland, California Telephone 1-800-227-1060 Description of Service Occupational Health Services, Inc., a professional counseling organization, is contracted by the Mid-Atlantic Federal Employee Counseling Services Consortium, which includes EPA, to monitor a toll-free hotline for Federal employees with any problems they may have at work or home such as marital, drug or alcohol abuse, and stress-related problems. If additional counseling is needed in person, OHS will refer the person to a counselor within the Philadelphia area. This is covered by the employee's health benefits. Also, the staff members of the Health Unit are competent to handle psychiatric emergencies which occur during working hours. Arrangements will be made for private psychiatric consultations. Availability: The counseling services are available to all employees at the 841 Chestnut building. Appointments: Appointments for further counseling services are arranged by the employee based upon OHS' referral. Fitness Basement, 841 Chestnut Street Telephone 215-597-6642; FTS 597-6642 Description of Service Med-Fit of America has been contracted to operate the Wellness Center. Med-Fit will be responsible for implementing the physical fitness program to the physical specifications of each individual. The program is a unique combination of health intervention and education programs, topics of interest, and a personalized health and fitness assessment before beginning the program. 17 ------- Equipment: Motorized treadmills, stationary bicycles, computerized rowers, stairclimbers, approximately 12 individual Nautilus-type exercise stations, hand-held dumbbell weights. Hours: 6:00-9:00 a.m.; 11:00-2:00 p.m.; 4:00-7:00 p.m. Availability: The Wellness Center is available to all Federal employees located in the 841 Chestnut Building. Restrictions Before an employee can join the Wellness Center, he or she has to complete a Wellness Center Enrollment and Interest Survey Form. When the form has been completed, the . employee has to submit it with a check for the first quarterly usage fee of $20. The fees are to be paid on a quarterly basis and are due on the 20th of the month preceding each calendar quarter (3/20, 6/20, 9/20, 12/20). Agency employees enrolling after the beginning of any calendar quarter will pay a prorated fee of $1.50 per week for the balance of the quarter. Following receipt of the enrollment form and check, the employee is sent a medical screening form along with instructions on how to complete the health screening process. Once the completed health screen and medical certification forms have been received, the employee will be scheduled for a fitness assessment with one of the Wellness Center professional staff. The results of these tests, along with information the staff obtains concerning the individual's exercise preferences, will be used to develop a personal exercise program specifically tailored to his or her needs and desires. The Wellness Center staff may have to place a restriction on the number of persons who can join based on the amount of interest generated. Exercise Classes Aerobics, yoga, slimnastics, jazzercise, stretch. Physical Fitness Counseling The Wellness Center will provide a wide variety of individual health and fitness services and programs, including testing, orientations, nutrition and weight control counseling, and recommendations for healthful recreational activities. 18 ------- REGION 4 Atlanta, Georgia Health Services Health Unit—Ground Floor 345 Courtland Street, N.E. Telephone (404) 347-7072; FTS 257-7072 Physicals Tests Given: EKG, chest X-ray, blood (full-scale series), pulmonary function, liver function, renal function, urinalysis, audiometry, eye, heart status. Availability: Physical examinations are available to approximately 80 percent of all employees in Region 4, especially employees who are in Field Safety Categories 1-4 and those over 40 years of age in all categories. Appointments: All employees who are active in Field Safety Categories 1-4 are required to establish an appointment for medical monitoring (physical examinations). AH employees who are 40 years and older may set an appointment for a complete physical examination. Call the Health Unit. Fees: None Screenings Tests Given: Glaucoma and visual acuity, blood pressure, cholesterol, sickle cell anemia, flu shots, diabetes. Availability: These screenings are available to all EPA employees and all contractor personnel. Appointments: Appointments are not required; service is on a first-come, first-served basis. Telephone (404) 347-7072; FTS 257-7072 Description of Service An Employee Assistance Program is available to provide counseling to EPA Region 4 employees. Counseling is available for a wide range of employee problems including drug and alcohol abuse and any emotional/behavioral problems which may adversely impact on job performance or behavior. Confidentiality is maintained in accordance with applicable regulations. Referrals may be either employee-initiated or supervisor-initiated. Emergency counseling is available on a 24-hour basis. Referral to private or community treatment resources will be made where appropriate. Services are provided by an external consultant, currently EMPACT, a division of Steve Watson and Associates. 20 ------- Availability: All EPA Region 4 employees can participate. Appointments: Counselors are available for a minimum of 12 hours per week. Appointments can be arranged through contacting the Health Unit at (404) 347-7072 or FTS 257-7072. On-site counseling is also provided at the EPA Environmental Services Division, College Station Road, Athens, Georgia, for a minimum of 4 hours per month. Fees: None Education Services Training sessions are offered periodically to employees on stress management or other similar topics. Orientation Orientation to the Employee Assistance Program is provided during new employee orientation training and to other groups of employees, supervisors, and managers when needed. Availability: Physicals are available to only a limited number of employees. An application with a questionnaire is distributed to all employees once a year. Employees are required to send the completed form to the EPA Safety Officer. The Safety Officer will exclude from selection any employees who (1) had a physical exam during the previous 24 months, (2) have physical exams covered by health insurance, and (3) are exposed to hazardous materials or waste (these employees are provided continuous medical monitoring by the Health Unit). After excluding the employees who do not qualify, the Safety Officer makes a selection using an index developed by incorporating such factors as age, health history, heredity, and job-related conditions (stress). Appointments: The Safety Officer will send the applications of the employees selected to the Health Unit. The Health Unit will contact the employees and schedule an appointment. Fees: None Screenings Tests Given: Hearing (upon request), visual acuity, tonometry, Pap smear, blood pressure (testing and monitoring), blood sugar, blood cholesterol. 22 ------- Availability: Health screenings are available to all employees. Blood pressure testing and monitoring are available continuously. Other screenings are announced by flyers distributed to all employees. Appointments: Employees must fill out the flyers and send them to the Health Unit. The Health Unit will contact employees and schedule an appointment. Fees: None Health Education Programs Subjects: Lectures including a wide variety of topics (sickle cell anemia, breast cancer, AIDS, blood pressure, office ergonomics, physical fitness) are offered. The Health Unit coordinates mental health presentations with the Employee Counseling Service. Availability: Seminars are held between noon and 1:00 p.m. on the dates indicated on flyers that are distributed to all employees. Allergy Shots Employees must bring in their serum. Health Unit personnel will administer the shots. The Health Unit also needs a signed authorization from the employee's local physician. Flu Shots Given to people 65 years or older and to people with chronic illnesses. Sick Call Employees who become ill or are injured during work may report to the Health Unit for help. Assistance is also offered to nursing mothers. 23 ------- Immunizations for Foreign Travel To obtain the shots on time, employees should contact the Health Unit as soon as they know they are going to a foreign country. Tetanus Shots Available on request to employees who have suffered a puncture wound. Emergencies In case of emergencies, someone from the Health Unit will try to determine if paramedics should be called immediately. They will provide first aid until paramedics arrive, or if paramedics are not needed. Counseling is provided by EPA contractors who are certified counselors. If a referral is needed, counselors will recommend appropriate services and will consider financial factors to assure that the services are affordable. Followup is provided to verify employees are receiving quality assistance.Use of the program is confidential. Information will not be released without prior written consent from the employee. Counseling records will not become part of an employee's official personnel file. 24 ------- Availability: All employees are eligible. Appointments: Can be made in person or over the phone. Fees: None 27 ------- Health Education Programs Subjects: Nutrition, weight management, behavior modification, stress management, cardiovascular risk reduction. Lectures including a wide variety of topics (sickle cell anemia, breast cancer, AIDS, blood pressure, office ergonomics, physical fitness) are offered. The Health Unit coordinates mental health presentations with the Employee Counseling Service. Availability: All employees. Seminars are held between noon and 1 p.m. Flyer announcements are distributed to all employees. Allergy Shots Employees must bring in their serum. Health Unit personnel will administer the shots. The Health Unit also needs a signed authorization from the employee's local physician. Sick Call Employees who become ill or are injured during work may report to the Health Unit for help. Prenatal Program Blood pressure testing, weight monitoring, and prenatal health education are provided to augment care provided by private physician. Pregnant women may lie down in the Health Unit for one hour per day with a note from their physician approved by their supervisor. Assistance is also offered to nursing mothers. Immunizations for Foreign Travel To obtain the shots on time, employees should contact the Health Unit as soon as they know they are going to a foreign country. Tetanus Shots Available on request to employees who have suffered a puncture wound. 28 ------- Counseling Through contract with PHS, Human Affairs International (HAI) Southland Center Dallas, Texas (214) 263-3001 Description of Service The service comprises short-term counseling as well as referrals. A 24-hour, toll-free number (1-800-624-6185) is professionally staffed at all times. The service is strictly confidential. Employees normally see an HAI counselor. The service provides assistance with alcohol and drug abuse, marital and family problems, stress or life crises, and legal and financial problems. Availability: All employees. Located in major cities throughout Region 6. Appointments: In person or by phone. Fees: None Fitness No programs. 29 ------- REGION 7 Kansas City, Kansas Health Services Business and Industry Health Group 30 Funston Road Telephone (913) 321-2420 Physicals Tests Given: Physical examination, blood studies, audiometry, EKG, tonometry (glaucoma), pulmonary functions, eye exam, Pap smear. Doctor administers test and does blood work on site. Availability: Ten percent of the workforce (40 people) are eligible for physicals. Selection of participants is based upon such factors as the date of the last physical (longest period without a physical is a priority). Requests for physical must be submitted by the employee in writing. Region 7 is required to make one half of the 40 slots available to bargaining employees, and the other half to nonbargaining employees. The final determination for participation is made by EPA Region 7. Appointments: Contact the Health Clinic. Fees: None Screenings Tests Given: Blood pressure, blood cholesterol, blood sugar, Pap smear, hearing, EKG, eye exam, pulmonary functions, blood chemistry profile, diabetes screening, glaucoma, hemoccult, immunization (influenza, tetanus), breast examination, protology exam, vision. Tests are given by nurses and assistants. Blood work is sent out for evaluation. Availability: All employees are eligible to receive this service. Some tests are offered daily and others on a more limited basis. Fees: None 30 ------- Health Education Programs Subjects: Topics of discussion at lectures include AIDS, stress, physical fitness, and proper nutrition. Availability: Employees are notified of "brown bag lunches" by public announcement (memos, etc.), monthly bulletin board postings, and the quarterly training schedule. The luncheons, which are held weekly, cover a wide range of topics. They do not always concern health matters. Allergy Shots If employees supply their serum, the Health Clinic will administer the shots. On-the-Job Illness Employees who become ill while at work may lie down at the Health Clinic for 1 hour. After this, they must take sick leave and go home or seek medical care. Pregnancy/Maternity Pregnant women may lie down to rest if they become ill while at work. Counseling Employee Assist Program Telephone 1-800-227-1060 Description of Service Counseling and self-help programs are available for both work and personal problems. Anything discussed in the counseling program is confidential. No information about an employee's use of the program may be released without prior written consent from the employee involved. Any information obtained from the counseling service cannot become a part of the employee's official personnel file. 31 ------- Availability: The service is paid by EPA and allows for four free visits to a local physician or appropriate counselor, designated by the program. Dependents and spouse of the employee are included in the coverage. Counseling services offered by the program include, but are not limited to, financial, psychiatric, marriage, weight control, substance abuse, and alcohol abuse. Appointments: Can be made in person or by phone. Fees: None Fitness A fitness program has not yet been started, but walking and weight loss are encouraged. The first two physicals are available to most employees within the following guidelines. 33 ------- 1. Employee health medical exam (EHME): given by Health Unit physician. 2. "Maxi" physical examination: given by Health Unit registered nurse. Tests Given: Complete blood studies, urinalysis, audiometry, EKG, tonometry (glaucoma), pulmonary function, visual acuity. Availability: EHMEs are available to 10 percent of EPA employees based on the Region's established age and medical priorities and on a first-come, first-served basis. Only one physical is permitted annually. Age and Medical Profile Priorities • Over age 40 with medical profile (i.e., diabetes, heart condition, etc.) • Under age 40 with medical profile. • Over age 40, no medical profile. • Under age 40, no medical profile. Maxis are encouraged since 20 percent of EPA employees are allowable. 3. Occupational safety and health employee exam (OSHEE): The Region requires separate ongoing physicals for employees who meet the requirements in Region 8's Medical Monitoring Protocol. This protocol limits the OSHEE to field and laboratory operative employees (i.e., on-scene coordinators and field inspectors). These employees are required to participate in the medical monitoring program. Appointments: Contact the PHS unit for an appointment. Screenings Miniscreening: Blood pressure, height, weight, urinalysis, blood workup. 34 ------- Availability: There is no Agency quota on the number of these mini-screenings given; therefore, employees are charged a lower contract fee for lab workups than if they were to have the work done through their own physicians. The employees are billed directly by the lab at the contracted price. Appointments: Call the Health Unit. Regional Office Miniscreening: Blood pressure, hearing, vision, diabetes. On Walk-in Basis: (PHS prefers that appointments be made) Allergy Shots Employees must bring in their own serum and instructions from their personal physician. Blood and Other Tests The tests in this category are for those employees who have been exposed to heavy metals. The availability of these tests is limited to field operative employees who are part of Region 8's budget. Flu Shots Open to all employees; no Agency quota. Tetanus, Cholera Immunization Immunizations for overseas travel, limited to those vaccines the PHS has available at the Health Units. Strep Tests Available at most PHS locations in Region 8. Pneumonia Shots Agency quota, based on criteria—age, medical history, etc. Measles Agency quota, based on criteria—age, medical history, etc. Emergency Services Call ambulance. Outreach Programs: Nurses go out in vans to areas outside of Denver where there are fairly large concen trations of Federal employees. They do various workups and make referrals to local doctors as necessary. Other Programs Tests Given: Treadmill, chest X-rays, and mammograms are at the individual's own expense with the exception of employees participating in the medical monitoring program (for field operative employees). Fees: None Fitness 999 18th Street 1 Denver Place Denver, Colorado 37 ------- Description of Services A coed Fitness Center recently opened and is located in temporary quarters on the Denver Place Mezzanine level (2nd floor) of the regional office until construction of its permanent location (to be completed after March 1989). The permanent location of the Fitness Center will be on the Mezzanine level, located in the Region's conference center (room 213). The center employs managers who provide some instruction on machine use and fitness counseling. Annual dues are charged on a sliding scale from $5.00 to $20.00, depending on the employee's grade level. Due to limited space, the Center is operated on an "Aerobic Super Circuit" concept during peak periods; i.e., usage is usually limited to 1/2 minute per machine/station with a "jogging pad" provided for running in place while waiting to use next machine. The Center is available from 6:00 a.m. to 6:00 p.m., with the doors opened from 10:00 to 2:00 and with key-card access the other 8 hours. The key cards would be furnished when the employee pays his/her annual dues and provides a medical certificate and liability waiver. For liability purposes, a visiting employee must furnish proof that he or she is already a member of his or her home region/Headquarters fitness center. The Employee Counseling Service is available to provide confidential counseling services for a variety of problems. OHS (Occupational Health Services, Inc.) is an independent organization providing assistance to employees and their immediate family members. EPA is one of the agencies participating in this program. Fees: The services provided by this program are contracted with Occupational Health Services, Inc. (OHS), and cannot be done on an individual basis. EPA Region 9 has contracted with OHS, paying $10.00 per person per year for these services. The Region has a corporate membership at the nearby YMCA providing all employees a discount membership. Due to the pending construction/renovation at the YMCA (anticipated in the summer of 1989), it is unclear at this time what services will be available under the corporate membership. The Region subsidizes annual memberships at the YMCA for all 25 field inspectors. Because their jobs require a great deal of physical effort and they are often required to wear a respirator, the Region requires the inspectors to go to the "Y" on a regular basis. Their participation and progress are monitored with this program. Health Education Programs Region 10 has an employee wellness program which is part of the Employee Assistance Program that offers health education programs. Subjects: The wellness program provides lectures, seminars and workshops on a monthly basis on various topics, including fitness, nutrition, stress management, burnout, financial planning, stretching, imagery and relaxation skills, weight training, AIDS, back problems. (Suggestions for additional topics are welcome.) In addition, workshops are scheduled on various topical issues once or twice a year. For example, the Health Promotion Program included a written lifestyle risk assessment, a play about stress in the workplace, and workshops on stress management, fitness, and nutrition.
