For facilities that dispose of wastewater without treatment, please identify the wastewater source
using the codes that were designated in the wastewater source questions in Sections B through
I (i.e., Questions B.9, C.13, D.3, E.2, F.2, G.4, G.20, H.2,1.3,1.14, and 1.24),
Columns B through F: Please enter the daily maximum, maximum monthly average, and long-
term average (over sample period) flows at all the sample points and wastewater sources listed in
Column A. Indicate the unit code and length of sample period.
Columns G through K: Please indicate the type/location of each sample point and wastewater
source listed in Column A.

This section requests detailed wastewater treatment system design
and cost information. This information will enable EPA to analyze
current wastewater treatment systems and to assess the economic
impacts of pollutant control technologies.
To identify each treatment unit in the following sections, use the
codes from Question J.11 that were assigned to the treatment units
in the Wastewater Treatment System Diagram, Question J.I4, page
J-31.
Capital costs for system installation, modifications, and upgrades
are requested in Table J-12.
This section applies only to the following types of wastewater
treatment systems.

Detailed Design Information
This section requests detailed wastewater treatment system design
and cost information. This information will enable EPA to analyze
current wastewater treatment systems and to assess the economic
impacts of pollutant control technologies.
To Identify each treatment unit in the following sections, use the
codes from Question J.11 that were assigned to the treatment units
in the Wastewater Treatment System Diagram, Question J.14, page
J~31.
Capital costs for system installation, modifications, and upgrades
are requested in Table J-12.
This section applies only to the following types of wastewater
treatment systems.

Detailed Design Information
Subsection 1
Equalization
This section requests design and maintenance cost information on
each equalization system used by your facility. Provide technical
design information, unless indicated otherwise. Identify non-design
(actual) information on the Comments page except where average
values are requested. Provide actual cost Information for 1989.
1.1. How many equalization tanks are on-slte?
I i I Equalization tanks
1.2. Do all equalization tanks have the same design basis?

tank design oasts. Complete Questions 1.4 through 1.20 for each different
equalization tank design basis. At the top of each page, Identify the design
basis with a number. Start with "01" for the first design basis and number the
remaining design bases sequentially.
Please note that tank size Is considered a design basis. Therefore, If your
facility has three different size equalization tanks, pages 1-5 through L-8 must
be completed three (3) times.
