PROCEDURES FOR INITIATING REMEDIAL
RESPONSE SERVICES
INTRODUCTION
Under both the REM/FIT and REM II contracts, all remedial response
activities will be initiated through work assignments issued to the
Contractor by the Contracting Officer. Exhibit 1 graphically illus-
trates the sequence of Federal and Contractor management interactions
and documentation required for initiation of Contractor remedial re-
sponse services through the issuance of a work assignment. Four basic
steps are involved.
Step 1 — Development and issuance of the work assignment
package
Step 2 — Completion of interim work assignment activities
Step 3 — Approval and implementation of the Contractor work
plan
Step 4 — Completion of work assignment amendments and
project closeout.
The following sections describe the procedures, paperwork, and person-
nel involved in implementing each of these steps.
STEP 1 — DEVELOPMENT AND ISSUANCE OF THE WORK ASSIGNMENT PACKAGE
The process begins in the Regional office where the Regional Site
Project Officer (RSPO) and/or the Regional Project Officer (RPO) will
prepare the work assignment package. The package will consist of four
elements:

A work assignment cover sheet
An interim work assignment statement of work (SOW)
A complete work assignment SOW for the entire project
A procurement request/requisition.
The work assignment cover sheet is a one-page summary of basic
information about the individual work assignment. It includes such
items as the name and address of the Contractor/ the contract number,
the name of the site, the work assignment number, the authorized level
of effort, the period of performance, and the names, addresses, and
phone numbers of the EPA Contracting Officer, Project Officer, RSPO,
and RPO. The RSPO and RPO must obtain the first two digits of the
work assignment number from the HQ Regional Coordinator since these
numbers are assigned sequentially. The last five digits are the
site-specific account numbers, which include:
A one digit activity code
A one digit Region identifier
A two digit Region-specific site/spill identifier
A decimal point and number to reflect an amendment to the
work assignment.
A copy of a sample work assignment cover sheet is shown in Exhibit 2.
The interim work assignment SOW defines the initial start-up tasks
the Contractor is expected to perform under the work assignment.
Typically, these include such things as the development of a work
plan, the collection and evaluation of existing data, the development
of a health and safety plan, the development of a QA/QC plan, topo-
graphic mapping, and development of a sampling plan. A copy of a
sample interim work assignment SOW is shown in Exhibit 3. The example
interim work assignment SOW shown also requests that the Contractor

should conduct a soil survey. In general, work such as a soil survey
or initial sampling may be performed under an interim SOW if such
efforts will promote the preparation of a more accurate and
comprehensive work plan. The interim work assignment SOW also defines
the authorized hours and dollars to be expended on the initial
start-up tasks. Exhibit 4 is provided as a guide for the RSPO/RPO to
use in preparing the interim SOW. The exhibit contains estimated
hours for professional level of effort (LOE), based on information in
existing work plans that have been developed for RI/FS activities at
sites that can generally be classified as either "simple", "moderate",
or "complex", with regard to the magnitude of the required remedial
response activities. These estimates are to be used as guidance only,
and do not necessarily represent upper or lower limits for
accomplishing a particular activity.
The complete work assignment SOW defines the remaining tasks the
Contractor is expected to perform under the full work assignment. The
SOW should be sufficiently detailed to define what must be done under
the activity, yet flexible enough to allow the Contractor some discre-
tion in developing an effective work plan to respond to EPA's needs.
The procurement request/requisition (PR) is used to order the
specific tasks and activities defined in the interim and final SOWs.
Guidance on completing a PR is forthcoming in an EPA publication en-
titled "Superfund Remedial Program Financial Management Authority,
Responsibilities and Procedures" which will be distributed to coincide
with the Regional delegations scheduled to take effect October 1, 1984.
When the four elements of the work assignment package are all com-
pleted, the RSPO/RPO will send the package to the Contracting Officer
with a copy to the HQ Regional Coordinator and HQ Project Officer.
Upon receipt of the package, the Contracting Officer will prepare a
contract modification using Standard Form 30 and then issue the work
assignment to the Contractor. The work assignment will contain both
the interim and complete SOWs.

STEP 2 — COMPLETION OF INTERIM WORK ASSIGNMENT ACTIVITIES
The Contractor will be required to acknowledge receipt of the work
assignment in writing within one week of receipt. The acknowledgement
of receipt must be submitted to the RSPO/RPO with copies forwarded to
the HQ Regional Coordinator and the Contracting Officer.
The interim work assignment SOW will stipulate that the Contractor
should prepare and submit within 15 days of receipt of the work as-
signment a schedule of interim work assignment SOW deliverables along
with an estimate of labor hours that will be required to produce the
work plan and each of the other interim deliverables. within 15 days,
the Contractor will submit this schedule and the estimate of required
labor hours to the RSPO/RPO for review and approval. If approved, the
RSPO/RPO will notify the Contractor to proceed with the task autho-
rized in the interim SOW. If not approved, the RSPO/RPO will contact
the Contractor to explain the reasons why and to discuss what modifi-
cations will be necessary to correct any problems with the scheduling
or labor hour estimates. The RSPO/RPO should pay particular attention
to the numbers of labor hours proposed by the Contractor to make sure
that these numbers do not vary substantially from those provided in
the interim work assignment authorization. Upon agreement between the
RSPO/RPO and the Contractor on any modifications, a new schedule or
labor hour estimate will be prepared by the Contractor and submitted
to the RSPO/RPO for final approval. If there is an increase in the
LOE above the amount provided in the interim authorization resulting
from the revisions, the approval must be submitted through the Con-
tracting Officer.
The Contractor will then initiate the interim SOW tasks according
to the schedule and labor hours approved by the RSPO/RPO. The de-
liver ables, as a result of conducting the initial tasks, may include,
but are not limited to, the following:

The most significant of these deliverables will be the development
of a detailed work plan, which describes how the Contractor plans to
accomplish all the activities and tasks outlined in the complete work
assignment SOW for the remedial response. It should address at least
the following elements:
Objectives and scope of the proposed activity and its rela-
tionship to other activities
Technical approach, anticipated problems, proposed solutions
Work to be performed under each task, including objectives
and scope, information sources, and methods to be used
. Safety and contingency measures (if applicable)
Schedule, including critical path and milestones, and staf-
fing of each task
Anticipated number of labor hours to complete the activity
A detailed cost estimate for the activity
Cash flow scheduling and cost control procedures
Subcontracting plan
Document production and distribution.

When the work plan is completed, the Contractor will submit it to
the RSPO/RPO for review and approval. If approved, the RSPO/RPO will
send the work plan to the Contracting Officer with copies to the HQ
Regional Coordinator and Project Officer. Attached to the work plan
will be a vork plan approval form, which is initiated by the.Contrac-
tor and submitted to the RSPO who completes the form, signs it, and
sends it to the RPO for approval and signature. The RPO then forwards
the form for review and approval to the Contracting Officer with
copies to the HQ Regional Coordinator and Project Officer. These
procedures may vary somewhat with respect to the actual processing of
the paperwork depending on the contractor involved. However, a
minimum number of approvals, that is, of the RSPO, RPO, and
Contracting Officer, will always be required. A copy of a sample work
plan approval form is shown in Exhibit 5.
If the RSPO/RPO does not approve the work plan, the RSPO/RPO will
contact the Contractor to explain the reasons for the disapproval and
to discuss what modifications in the work plan will be necessary to
correct the problems. Once the modifications are completed, the
RSPO/RPO will prepare and sign the work plan approval form and submit
it along with the work plan to the Contracting Officer with copies to
the HQ Regional Coordinator for review, processing and approval. The
total funding specified in the work plan for the project must be
within that of the original PR. If not, a PR for the incremental
increase must accompany the work plan approval form, or only partial
approval can be provided pending an increase in funding. Any
increases in funding in the PR must be consistent with Remedial
Accomplishments Plan (RAP) resource ceilings.
STEP 3 — APPROVAL AND IMPLEMENTATION OF THE CONTRACTOR WORK PLAN
Upon receipt of the work plan and the work plan approval form, the
Contracting Officer will review the work plan and either approve it or
reject it. In some cases, approval may be conditional, pending

changes in the budget, for example. This will be indicated on the
work plan approval form. The Contracting Officer or the HQ Regional
Coordinator will notify the RSPO/RPO of any problems which need to be
addressed in order to make the work plan acceptable and attempt to
resolve these problems with the RSPO/RPO. In those instances where
modifications to the work plan are necessary, the RSPO/RPO will
discuss the suggested revisions with the Contractor outlining the
concerns of the Contracting Officer and/or the HQ Regional
Coordinator. Upon agreement on the necessary changes, the Con-
tractor will prepare and submit to the RSPO a revised work plan which
addresses the Contracting Officer's and/or HQ Regional Coordinator's
commen ts.
Once the Contracting Officer and the HQ Regional Coordinator ap-
prove the work plan, they will sign the work plan approval form and
return it to the Contractor. The RSPO/RPO will be notified when the
work plan has been approved.
STEP 4 — COMPLETION OF WORK ASSIGNMENT AMENDMENTS AND PROJECT CLOSEOUT
After notification of work plan approval, the Contractor will pro-
ceed with the work. If, during the course of the work, it becomes
necessary to modify the complete work assignment SOW, an amendment to
the work assignment will have to be prepared by the RSPO, approved by
the RPO, and forwarded to the Contracting Officer and the HQ Regional
Coordinator, Deputy Project Officer, and Project Officer for final
approval,and issuance. This might be necessary because of a change in
technical direction, a change in the schedule for completion, or a
significant change in the amount of resources required to complete the
job.

The procedure that will be followed in preparing a work assignment
amendment will be as follows:

The Contractor will notify the RSPO of the proposed modifica-
tions. The Contractor must submit a revised Form 60 and pro-
vide a detailed explanation on a task by task basis of
changes (either increases or decreases) in LOE, cost, and
schedule.
The RSPO will review the proposed modifications and either
approve or disapprove them. If not approved, the RSPO will
contact the Contractor, explain why the proposed modifica-
tions are not approved, and attempt to come to an agreement
about what changes will be required. The RSPO will then com-
plete a work assignment amendment form (see Exhibit 6 for a
copy of a sample work assignment amendment form) and submit
it to the RPO for approval and signature.
The RPO will review the work assignment amendment form and
either approve or disapprove it. If it is not approved, the
RPO will notify the RSPO of the reason for the disapproval
and attempt to resolve the problem. If necessary, the RSPO
will contact the Contractor to make any necessary changes in
the proposed modifications to satisfy the suggestions and
.requirements of the RPO. The RPO, upon approval, will sign
the work assignment amendment, prepare a new PR and prepare a
RAP amendment if additional funds are needed which exceed the
current RAP ceiling limits, and submit them to the HQ
Regional Coordinator for review and approval.
If the HQ Regional Coordinator has any problems with the pro-
posed modifications in the work assignment amendment, he/she
will contact the RSPO or RPO to discuss and resolve them. In
those cases where the proposed modifications are significant,
the Coordinator should consult with the HQ Project Officer to
obtain his/her review and approval. The HQ DPO/Project Of-
ficer will then sign the work assignment amendment incorpora-

ting and authorizing all the approved modifications, and sub-
mit it to the Contracting Officer for approval. The Contrac-
ting Officer will then sign the amendment and issue it to the
Contractor. A copy of the work assignment amendment should
be sent by the Contracting Officer to the HQ Regional
Coordinator and the RSPO and RPO for their records.
The RSPO or RPO should also ensure that each approved work
assignment amendment is recorded on Page 2 of the Work As-
signment Completion Report (WACR) . Specific instructions for
preparing the amendments section of the WACR can be found in
the award fee performance evaluation plans, that are
available for both the RE^FIT and REM II contracts.
At the completion of the work assignment, the Contractor will pre-
pare a WACR (see Exhibit 7) and submit it to the RSPO and RPO who, in
turn, will submit it to the HQ Regional Coordinator and HQ Project
Officer for review and use in determining award fee recommendations.
The Contractor will also prepare and submit a final invoice and a work
assignment closeout form.* A sample copy of a closeout form is shown
in Exhibit 8. The RSPO will then fill out the closeout form,
indicating whether the work assignment is completed and the project
closed or whether it is incomplete and requires additional work, and
sends it along with the invoice to the RPO for signature. The RPO
will then return the closeout form to the Contractor who will retain a
copy for the Contractor file and submit the original to the HQ Project
Officer with copies to the HQ Regional Coordinator and the Contracting
Officer. The RPO will also send the final invoice to the HQ Project
Officer.
* If the work assignment involves the completion of an activity that
must be cost shared by the State (i.e., initial remedial measure), the
Contractor may also be required to submit reports (e.g., subcontractor
invoices) that document the completion of the remedial action. For
further guidance on the types of documentation that may be required,
refer to the U.S. EPA manual on State Participation in the Superfund
Remedial Program (February 1984).
