dioxide process line (hours)
Annual production of carbon-
containing waste (tons), if applicable
If facility generates carbon containing wastes:
       Monthly production of carbon
       containing waste (tons)
Annual sample of carbon content for
carbon containing waste (percent by
weight)
                                If using a CEMS:
 In addition to the monitoring requirements under 40 CFR subpart C for the
         Tier 4 Calculation Methodology, measure these parameters...
