Introduction The U.S. Environmental Protection Agency (EPA) prepared this List of Frequently Asked Questions (FAQs) to assist respondents with responding to the Meat and Poultry Products (MPP) Questionnaires. If you have questions that are not addressed in these FAQs, contact the EPA MPP Questionnaires Helpline (Helpline) for which contact information is provided in Table 1. Table 1. General Questionnaire I made a mistake on my submitted questionnaire and would like to revise one or more of my answers. What should I do? If you responded in Qualtrics and need to revise answers, contact the Helpline (see Table 1) for assistance revising your submission. If you mailed your hardcopy questionnaire and need to revise one or more answers, also contact the Helpline. Depending on the extent of these changes helpline staff will advise on the best method for communicating these revisions. Do not send any confidential business information (CBI) via email. If your revisions include CBI, you may contact the Helpline via email and request staff contact you by phone to discuss the CBI details of the revision. My facility underwent upgrades or operational changes during calendar year 2021. Do I still need to respond to the Questionnaire? As long as your facility meets the eligibility criteria for the MPP Questionnaires following the completion of any changes and/or upgrades, then a questionnaire response for your facility is still required. Respond with the most recent and complete information available for operations at your facility in 2021. My facility was offline (for renovation, upgrades, etc.) during all or part of calendar year 2021. Is my facility still eligible? How should I respond to the Questionnaire? As long as your facility has not permanently closed or ceased operation as of January 1, 2021, a questionnaire response for your facility is still required. If your facility was operating for any portion for 2021, respond with this information. Qualtrics will prompt you to skip inapplicable questions or enter zero (0) where appropriate to complete your response. 1 ------- 3. Accessing Qualtrics I accidentally discarded my facility's notification letter. What should I do? The notification letter mailed by EPA contains the unique Qualtrics entry code, known as the Facility ID, and the version of the questionnaire assigned to your facility. Contact the Helpline (see Table 1), and staff will assist you in recovering the information needed to access your facility's Qualtrics questionnaire. / received the following error message when I entered my Facility ID, "This survey has already been completed." The Facility ID you entered is associated with a questionnaire that has already been submitted in Qualtrics. Confirm you entered the correct Facility ID or try again. If you believe your facility's questionnaire has been submitted prematurely or in error, contact the Helpline (see Table 1). / received the following error message when I entered my Facility ID, "We do not recognize this Facility ID. Please check your entry and try again. If you believe this Facility ID is correct, please contact the EPA MPP Questionnaire Helpline for assistance." The Facility ID you provided is likely incorrect. Confirm you entered the correct Facility ID or try again. If the error persists, contact the Helpline (see Table 1). The facility name and/or address in my notification letter from EPA is incorrect. You will have an opportunity to correct this information in the questionnaire; you do not need to contact the Helpline or EPA. Proceed through Qualtrics until reaching Question 1, where the name and address of the facility can be corrected. EPA will use questionnaire responses to update contact and address information for MPP facilities. 4. Navigating Qualtrics What devices can I use to respond to the Qualtrics questionnaire? Qualtrics supports both mobile and desktop devices (e.g., mobile phone, tablet, laptop, desktop computer). However, EPA recommends using desktop devices as the rendering of some questions may be difficult to navigate on a small screen. / received the following error message when responding in Qualtrics, "Your session was outdated, we have updated you to the most recent response." There is likely another person(s) working in your facility's Qualtrics questionnaire. Close your browser and coordinate with other collaborators on a timed work schedule or relay system. Simultaneous collaboration can cause response data to be deleted and questions to be unintentionally skipped. Each collaborator must provide their responses when no other respondents are working on that questionnaire to ensure data are saved properly. / did not print a copy of my responses before submitting my Qualtrics questionnaire. How can I obtain a copy of my responses for my records? Contact the Helpline (see Table 1). If you responded in Qualtrics, staff can provide a linkfrom which you may access this page in Qualtrics and print again. If you responded via hardcopy, staff can coordinate mailing you a copy of your response. 2 ------- How will I know EPA has received my questionnaire submission in Qualtrics? After you submit your facility's Qualtrics questionnaire, you will receive a message in your browser window confirming the date and time your response was submitted to EPA. This message will confirm it is now ok to close your browser window. You will also receive a confirmation email from Qualtrics documenting the date and time your questionnaire was submitted to EPA. This email will be sent from noreply@qemailserver.com. If you did not receive this email, check your junk and SPAM folders and then contact the Helpline (see Table 1) to be sent a copy of this email. 5. Hardcopy Questionnaires How can I obtain and/or print a hardcopy questionnaire ? Printable PDFs of the MPP Questionnaires are available on EPA's MPP Questionnaire website (https://www.epa.Rov/eR/mpp-questionnaires). If your facility lacks internet access, contact the Helpline (see Table 1) to inform staff of your need to respond outside of Qualtrics and have questionnaire materials mailed to your facility. How will I know EPA has received my hardcopy questionnaire submission? Hardcopy questionnaires must be submitted, along with attached files, to the Helpline (see Table 1) by postal mail. Electronic files should be included in the mailed package on removable media (e.g., CD, flash drive). EPA recommends mailing your submission using a trackable mailing service to ensure delivery. 6. Questionnaire Content Qualtrics says my file is too big to upload. What should I do? Qualtrics will not accept files larger than 100 MB for upload. For files larger than this limit, zip or compress your file and try to upload the smaller file (instructions on how to zip files are included in the General Instructions). If zipping or compressing the file does not sufficiently reduce the file size, or the file is unable to be compressed, mail your file on removable media (e.g., CD, flash drive) to the Helpline (see Table 1). If you need to proceed in Qualtrics without uploading a file because you are submitting it by mail, contact the Helpline. Qualtrics says my file is not the correct type/does not have the correct extension. What should I do? Qualtrics is programmed to only accept certain files for individual questions. Instructions on which file types and extensions are acceptable for upload are included with every applicable question; read these instructions carefully. Qualtrics will prevent the upload of files not in these specified types. Convert files as needed into acceptable file types; contact the Helpline (see Table 1) if you need assistance or instruction on converting file types. How do I make an aerial map of my facility campus? / Can I use an existing aerial photograph to make a map? You may use an existing aerial photo or map of your facility. You may also use a screenshot from a satellite imagery application, such as Google Maps™. You may hand-draw labels on the map or use computer software to add these descriptions. 3 ------- Where do I get the template files I need? / How do I use the template files? The Detailed Questionnaire includes specific questions asking respondents to provide responses using template Excel workbooks. These template files are available for download from the EPA MPP Questionnaires website and from within Qualtrics with each applicable question. Each template Excel workbook contains a ReadMe spreadsheet that includes instructions on how to complete each file. There is also an Example Data spreadsheet in each file that uses sample data to demonstrate how to complete the workbook. 4 ------- |
