Authorized Agency: Agency authorized to implement the program (state, local, or EPA)
Review Year: Fiscal year in which the activities being reviewed were conducted
Statutes Covered:

o Clean Water Act (CAA) - National Pollutant Discharge Elimination System (NPDES)
o Clean Air Act (CWA) - Title V

o Resource Conservation and Recovery Act (RCRA) - Subtitle C
Source Information:

o Data Metrics - Verified compliance and enforcement data in the national data systems
o File Metrics - Facility files that contain compliance and enforcement activity
o Other - Non-review year data or multi-year data trends; results of previous SRF reports;

performance agreements; follow-up conversations with agency personnel
Program Elements Covered:

o Data - Completeness, accuracy, and timeliness of data entry into national data systems
o Inspections - Meeting inspection and coverage commitments, inspection report quality,
and report timeliness

o Violations - Identification of violations, accuracy of compliance determinations, and

determination of significant noncompliance (SNC) or high priority violators (HPV)
o Enforcement - Timeliness and appropriateness of enforcement, returning facilities to
compliance

o Penalties - Calculation including gravity and economic benefit components, assessment,

and collection
Finding Levels:

o Meets or Exceeds: No deficiencies were identified, the base level of program

performance was met or exceeded,
o Area for Attention: Minor deficiencies were identified. One or more metrics indicates
deficiencies in performance related to program activities, processes, or policy. The
authorized program is to correct the issue without additional EPA oversight. EPA may
make suggestions to improve performance, but they will not be monitored for completion
between SRF reviews,
o Area for Improvement (Recommendation Required): Significant deficiencies were
identified. One or more metrics indicates deficiencies in performance related
to program activities, processes, or policy. Recommended actions to correct the issue are
included in the report. Recommendations must have well-defined timelines and
milestones for completion, and, if possible, should address root causes. EPA will monitor
recommendations for completion between SRF reviews
Recommendation Status

o Ongoing: The recommendation is in progress, it is expected to be completed, and it is on

schedule to meet the agreed upon completion date
o Completed: The requirements of the recommendation have been met and all

documentation, if appropriate, has been provided to verify implementation
o Overdue: The recommendation has not been completed by the designated due date
