Fact Sheet S2 E PA underground injection control Class V - Injection Well Information Aquifer Remediation Injection Wells for Air Sparging The Underground Injection Control (UIC) Program, created under the authority of the Safe Drinking Water Act (SDWA), is a preventative program aimed at protecting existing and future underground sources of drinking water (USDWs). Shallow wells or disposal systems that discharge fluids into the subsurface are known as Class V wells and can be authorized to inject by rule or permit. Class V wells that have the potential for ground water contamination or degradation usually operate under a UIC Permit. Those that do not have a potential to contribute to contamination or degradation of ground water are usually rule authorized, once EPA has been provided with information according to the requirements of 40 CFR 144.26 and 144.27. Injection of oxygenating compounds falls under the regulatory authority of the Class V program, because of the definitions under 40 CFR 144.3 of which define "Well: a bored drilled or driven shaft whose depth is greater that the largest surface dimension or a subsurface fluid distribution system;" and "Fluid: any material or substance which flows or moves whether in a semi-solid, sludge, gas or any other form or state." The UIC Program Director has determined that, in general, injection of oxygenating compounds do not have the potential to endanger USDWs, and can, therefore, Authorized by Rule once the following information is provided to EPA. Please provide the following information: • Property owner of facility including a physical and mailing address; phone and fax numbers. • Operator of facility including a physical and mailing address; phone and fax numbers. • Responsible party for the operation, maintenance, and closure of the injection system including a physical and mailing address; phone and fax numbers. • Name of the facility • Map of the site & well location or general area where injection wells will be located. • Is this a proposed or existing system? • Name, address and phone number for contact in the Colorado State Agency authorizing site clean up. • Briefly outline the type of contamination and when it occurred.
