Class V - Injection Well Information
Injection of Tracers

The Underground Injection Control (UIC) Program, created under the authority of the Safe Drinking
Water Act (SDWA), is a preventative program aimed at protecting existing and future underground sources of
drinking water (USDWs). Shallow wells or disposal systems that discharge fluids into the subsurface are
known as Class V wells and can be authorized to inject by rule or permit. Class V wells that have the potential
for ground water contamination or degradation are usually permitted. Those that do not have a potential to
contribute to contamination or degradation of ground water are usually rule authorized, once EPA has been
provided with information according to the requirements of 40 CFR 144.26 and 144.27. Injection of tracers for
ground water studies falls under the regulatory authority of the UIC Class V program because of the following
definitions under 40 CFR 144.3: "Well: a bored drilled or driven shaft whose depth is greater that the largest
surface dimension or a subsurface fluid distribution system;" and "Fluid: any material or substance which flows
or moves whether in a semi-solid, sludge, gas or any other form or state." In general, the UIC Program Director
has determined that, in general, injection of tracers does not have the potential to endanger USDWs, and can,
therefore, be authorized by rule once the information listed below is provided to EPA. However, there is the
concern when bromide solutions are injected up-gradient from a Public Water System supply well. When
bromide is treated with ozone or CLO3, bromide is converted to BrO3".

D Operator of facility including mailing address, phone and fax numbers.

D Responsible party for the operation, maintenance, and closure of the injection system including address,
phone and fax numbers.

~	Name of the facility and physical location.

D Map of the site including injection well locations or general area where injection will occur, and proposed or
existing monitoring wells.

~	Are existing wells being used for injection or will new wells be constructed?

D Type of injection wells to be used for injection, (example: water supply well, monitoring well, etc.)

~	Name, address and phone number for contact at the Colorado State Agency overseeing the project.
D Description of the proposed tracers and purpose for tracer study.

D Hydrogeologic description, location, depth, and current use (if any) of the receiving formation(s).

~	Note the location of any public water supply wells completed in the receiving formation(s).

~	Estimation of time frame for when injection activities will begin and end.
