OPERATOR RESPONSIBILITIES

MECHANICAL INTEGRITY FAILURES

1) IMMEDIATELY - Cease injection and shut-in the well as rapidly as feasible. In
no case shall the well remain in operation beyond 48 hours unless the UIC
Technical Enforcement Office Director allows temporary operation of the well.

2) WITHIN 24 HOURS - Verbally notify the UIC Technical Enforcement Office of
the MIT failure, even in cases where the failure is detected during a test which
was witnessed by a UIC inspector.

3) WITHIN 5 DAYS - Submit a written follow-up report documenting circumstances
of the failure, any test results, remediation actions taken and results, or any
proposed remediation plan within limits established by the Director on
appropriate volume, pressure, or time limitations for continued injection
operation.
