1.	LOCATIONS OF ALL EXISTING STRUCTURES, SUBSURFACE SOIL OR ROCK CONDITIONS ARE
BASED ON THE BEST AVAILABLE INFORMATION AND NOT WARRANTED TO BE EXACT, NOR IS IT
WARRANTED THAT ALL ARE SHOWN, UTILITIES ARE KNOWN TO BE PRESENT ON THE SITE ALONG

AS INFORMATION PRESENTED BY THE CONTRACT DRAWINGS AND SPECIFICATIONS.

3.	THE CONTRACTOR IS RESPONSIBLE FOR LOCATING, INSPECTING, AND PROTECTING ALL
EXISTING SITE IMPROVEMENTS OR STRUCTURES. IT SHALL BE THE CONTRACTOR'S
RESPONSIBILITY TO SATISFY THEMSELVES THAT ALL THE EXISTING SITE IMPROVEMENTS,
WHETHER SHOWN ON THESE CONTRACT DRAWINGS OR NOT, HAVE BEEN PROPERLY LOCATED
AND PROTECTED.

4.	THE CONTRACTOR IS RESPONSIBLE FOR ALL PROJECT SAFETY INCLUDING, BUT NOT LIMITED
TO, EXCAVATION, TRAFFIC CONTROL, AND SECURITY.

5.	THE U.S. ENVIRONMENTAL PROTECTION AGENCY (EPA) WILL OBTAIN ALL ACCESS AGREEMENTS
NECESSARY TO ACCESS THE PROPERTIES THAT REQUIRE REMEDIATION. THE CONTRACTOR IS
NOT TO PERFORM WORK ON ANY PROPERTIES WITHOUT AN ACCESS AGREEMENT.

6.	THE CONTRACTOR SHALL LIMIT THEIR ACTIVITIES TO WITHIN THE WORK AREAS SHOWN ON THE
CONTRACT DRAWINGS. WORK CONDUCTED OUTSIDE OF WORK AREAS, IF REQUIRED, SHALL BE
COORDINATED WITH EPA.

7.	THE CONTRACTOR SHALL RESTORE ALL DAMAGED AREAS TO THEIR ORIGINAL CONDITION.

8.	THE TERM 'ACCEPTABLE' SHALL MEAN WRITTEN ACCEPTANCE TO BE RECEIVED FROM EPA
BEFORE WORK IS STARTED.

9.	ANY DEVIATIONS FROM THE CONTRACT DRAWINGS OR SPECIFICATIONS MUST BE ACCEPTED, IN
WRITING, PRIOR TO THE WORK BEING DONE. ANY DEVIATIONS PERFORMED WITHOUT
ACCEPTANCE WILL NOT BE PAID FOR, AND MAY BE REQUIRED TO BE REDONE AT THE
CONTRACTOR'S EXPENSE.

10.	THE CONTRACTOR SHALL HAVE A COPY OF THE CONTRACT DOCUMENTS AT THE WORK SITE
AT ALL TIMES.

11.	SEE INDIVIDUAL SHEETS FOR ADDITIONAL ABBREVIATIONS, SYMBOLS, LEGENDS, DETAILS AND
NOTES, IF ANY. NOT ALL 'STANDARD' DETAILS SHOWN MAY BE APPLICABLE TO THIS PROJECT.

12.	UNLESS NOTED OTHERWISE, ALL WORK SHOWN IN THESE CONTRACT DOCUMENTS IS NEW
AND TO BE PERFORMED UNDER THIS CONTRACT.

13.	THE CONTRACTOR SHALL BE RESPONSIBLE FOR MAINTAINING SITE SECURITY FOR THE
CONTRACTOR'S FACILITIES AND EQUIPMENT WITHIN THE LIMITS OF THE CONTRACT.

14.	THE CONTRACTOR SHALL EXERCISE CAUTION WHEN WORKING AROUND MINE SHAFTS AND
SUBSIDENCE PITS, SUBSURFACE VOIDS EXIST IN THE IMMEDIATE AREAS WHERE THE EXCAVATION
AND BACKFILLING ACTIVITIES ARE BEING PERFORMED. THESE VOIDS MAY COLLAPSE OR OPEN AT
ANY TIME. THE CONTRACTOR SHALL SUBMIT A SUBSIDENCE PIT AND MINE SHAFT FILLING PLAN IN
ACCORDANCE WITH SECTIONS 02111 AND 02220.

15.	THE SIDES OF THE VERTICAL OPENINGS AND SUBSIDENCES MAY BE EXTREMELY UNSTABLE.
EXTREME CAUTION SHOULD BE USED WHEN WORKING AROUND ALL WORK AREAS TO PREVENT
PERSONNEL AND/OR EQUIPMENT FROM FALLING INTO THE HOLES. THE CONTRACTOR IS SOLELY
RESPONSIBLE FOR WORKING SAFELY AT ALL AREAS OF THE SITE. THE CONTRACTING OFFICER'S
REPRESENTATIVE WILL STOP WORK IF THE CONTRACTOR IS NOT FOLLOWING GENERAL SAFETY
PRACTICES.

16.	EXCAVATION AND BACKFILL ACTIVITIES SHALL BE PERFORMED IN ACCORDANCE WITH THE
REQUIREMENTS OF THE PERFORMANCE WORK STATEMENT, SPECIFICATIONS, AND THE
CONTRACTOR'S APPROVED SITE HEALTH AND SAFETY PLAN. APPROPRIATE HEALTH AND SAFETY
MEASURES AND TRAINING WILL BE REQUIRED AT ALL WORK LOCATIONS.

17.	THE TERM 'MINE WASTE' SHALL MEAN CHAT, TAILINGS, AND CONTAMINATED SOIL WITH METALS
CONCENTRATIONS EXCEEDING CLEANUP GOALS SPECIFIED IN THE PERFORMANCE WORK
STATEMENT.

18.	COORDINATES USED ON CONTRACT DRAWINGS ARE NAD-83 (CORS 96) KANSAS STATE PLANE,
ZONE 1502 KANSAS SOUTH IN U.S. SURVEY FEET. VERTICAL DATUM IS MEAN SEA LEVEL NAVD
1938.

19.	SITE TOPOGRAPHIC SURVEY AND AERIAL PHOTOGRAPHY PERFORMED BY AERIAL DATA
SERVICES, APRIL 2013. THE FLIGHT HEIGHT OF THE AERIAL PHYOTOGRAPHY WAS 2,000 FEET
ABOVE MEAN TERRAIN ELEVATION. NO PERMANENT BENCH MARKS OR REFERENCE POINTS WERE
ESTABLISHED FOR THE SITE TOPOGRAPHIC SURVEYS.

8.	CONFIRMATORY SAMPLING SHALL BE CONDUCTED BY THE CONTRACTOR IN
ACCORDANCE WITH SECTION 02111.

9.	RESTORATION ACTIVITIES, INCLUDING BACKFILLING, SHALL NOT COMMENCE
WITHOUT APPROVAL BY EPA.

10.	THE CONTRACTOR SHALL EXCAVATE AROUND TREES AND BUSHES TO BE LEFT IN
PLACE UNLESS OTHERWISE SPECIFIED, IN ACCORDANCE WITH SECTIONS 01490 AND
02111. THE EXCLUSION RADII FOR TREES AND BUSHES LEFT IN PLACE ARE SPECIFIED
IN SECTION 02111.

11.	OVEREXCAVATION OF UNIMPACTED SOIL WITHIN THE MINE EXCAVATION AREAS TO
GENERATE COVER MATERIALS FOR THE CONSOLIDATION AREAS IS STRICTLY
PROHIBITED EXCEPT FOR AREAS NOTED ON THE DRAWINGS.

12.	THE NORTHWEST TRIBUTARY OF TAR CREEK EXTENDING FROM THE LARGE POOL
OF WATER EAST OF AREA 2 TO STAR ROAD WILL BE REMEDIATED UNDER THIS
CONTRACT. THE TRIBUTARY IS CLASSIFIED AS AN EPHEMERAL STREAM
CHARACTERIZED BY A RELATIVELY NARROW INCISED CHANNEL WITH STEEP BANKS
AND STANDING WATER ALONG MOST OF ITS LENGTH. CHAT AND OTHER TYPES OF
MINE WASTE ARE VISIBLE ALONG THE BOTTOM OF THE CHANNEL DUE TO RUNOFF
FROM THE SURROUNDING MINE WASTE AREAS.

13.	REMEDIATION OF THE CREEK CHANNEL AND SURROUNDING AREA(S) WILL BE
PERFORMED BASED ON THE CONTRACT REQUIREMENTS AND THE CONTRACTOR'S
NORTHWEST TRIBUTARY RESTORATION PLAN AS APPROVED BY EPA. REFER TO THE
PERFORMANCE WORK STATEMENT FOR SPECIFIC REQUIREMENTS FOR THIS PLAN.

14.	IN GENERAL, THE REMEDIATION CONCEPTS FOR THE NORTHWEST TRIBUTARY OF
TAR CREEK INCLUDE REMOVAL OF 18 INCHES OF EXISTING MINE WASTE AND SOIL
BENEATH THE EXISTING CHANNEL BOTTOM AND DISPOSAL OF THE EXCAVATED
MATERIALS WITHIN THE DESIGNATED WASTE CONSOLIDATION AREAS. WHERE THE
DEPTH OF EXISTING MINE WASTE IS GREATER THAN 18 INCHES, THE DEPTH OF
EXCAVATION SHALL BE INCREASED TO REMOVE THE VISIBLE MINE WASTE.
REMEDIATION OF IMPACTED SOIL WITHIN THE FLOODPLAIN ADJACENT TO THE CREEK
CHANNEL SHALL BE BASED ON EXCAVATION TO ATTAIN THE SOIL REMEDIATION
STANDARDS PROVIDED IN THE PERFORMANCE WORK STATEMENT.

15.	THE DEPTH OF EXCAVATION BENEATH EXISTING BRIDGES ALONG THE EXISTING
CHANNEL SHALL BE LIMITED TO REMOVAL OF VISIBLE MINE WASTE TO PREVENT
UNDERCUTTING AND INSTABILITY OF EXISTING FOUNDATIONS. REFER TO THE
PERFORMANCE WORK STATEMENT FOR ADDITIONAL INFORMATION. WORK IN THE
VICINITY OF THE EXISTING BRIDGES SHALL BE COORDINATED WITH EPA PRIOR TO
START OF EXCAVATION TO ALLOW DIRECT OBSERVATION OF EXPOSED BRIDGE

4.	TEMPORARY PROJECT SAFETY FENCING SHALL BE CONSTRUCTED AROUND WORK
AREA(S) AS DIRECTED BY EPA AND AS SPECIFIED IN THE PERFORMANCE WORK
STATEMENT.

5.	EROSION AND SEDIMENT CONTROLS SHALL BE REMOVED UPON ACCEPTANCE OF
VEGETATION AS SPECIFIED IN SECTION 02921.

GRADING AND RESTORATION PLAN NOTES:

1.	RESTORATION SHALL BEGIN ONLY AFTER CLEANUP GOALS IN AN EXCAVATION

7.	BACKFILL ON WET SUBGRADE AS A RESULT OF GROUNDWATER MAY BE
ENCOUNTERED AT SHALLOW DEPTHS ADJACENT TO THE NORTHWEST TRIBUTARY
AND PONDS. CONTRACTOR MAY NEED TO PLACE BACKFILL IN WET AREAS. BACKFILL

8.	THE SITE AREAS MAY CONTAIN FENCING FOR SECURITY AND LIVESTOCK CONTROL.

EXCAVATION PLAN NOTES:

(REFER TO SPECIFICATION FOR ADDITIONAL INFORMATION)

1.	THE AREA 2 (TX-33) MINE WASTE AREA IS CURRENTLY USED TO GRAZE LIVESTOCK AND IS LOCATED ADJACENT TO
CULTIVATED FIELDS. THE SITE CONTAINS DISCONTINUOUS AREAS OF SHALLOW CHAT AND OTHER MINE WASTE MIXED
WITH SOIL, LARGE AREA OF CHAT, TAILINGS, AREAS OF CONCRETE RUBBLE, AND VENT PIPES. THE CONTRACTOR
SHALL COORDINATE REMEDIAL ACTIVITIES WITH THE PROPERTY OWNERS AND PROVIDE TEMPORARY FENCING FOR
LIVESTOCK CONTROL, AS NEEDED, DURING THE REMEDIAL ACTIVITIES.

2.	SITE ACCESS ROAD IS A GRAVEL TRAIL LOCATED ON THE SOUTHEASTERN EDGE OF THE LIMITS OF CONSTRUCTION
AS INDICATED ON THE CONTRACT DRAWING. THE GRAVEL TRAIL IS OFF SW20TH STREET. CONTRACTOR SHALL MAKE
IMPROVEMENTS TO THE ROAD AS NECESSARY. TRAFFIC CONTROL SHALL BE USED FOR ALL TRUCKS AND EQUIPMENT
ENTERING AND EXITING SITE IN ACCORDANCE WITH SECTION 02121. REFER TO NOTES ON SHEET G-03 REGARDING
THE CONTRACTOR'S RESPONSIBILITY FOR ANY DAMAGES TO THE EXISTING BRIDGE ON SW20TH STREET AND THE
BRIDGE ALONG THE EXISTING FARM TRAIL ACCESS ROAD TO AREA 2 LOCATED WEST OF SW20TH STREET.

3.	AREA 2 IS LOCATED ADJACENT TO AREA 1. THE ABANDONED RAILROAD SHALL BE USED AS A TEMPORARY HAUL
ROUTE FROM AREA 1 TO THE AREA 2 MINE WASTE CONSOLIDATION AREA.

4. CONDUCT XRF ANALYSIS (FIELD XRF SCREENING WITH PORTABLE INSTRUMENT) WITHIN THE INITIAL
CONSTRUCTION LIMITS WHERE VISIBLE MINE WASTE IS NOT PRESENT AND OUTSIDE THE PROPOSED LIMITS OF
THE MINE WASTE CONSOLIDATION AREA TO IDENTIFY AREA(S) REQUIRING REMEDIATION. XRF SCREENING
SHALL BE CONDUCTED ON A 50-FOOT GRID IN ACCORDANCE WITH THE APPROVED QUALITY ASSURANCE PLAN
AS REQUIRED BY THE PERFORMANCE WORK STATEMENT. PROVIDE EPA WITH A SITE PLAN SHOWING THE GRID
LOCATIONS, XRF RESULTS, AND INTERPRETED AREAS REQUIRING EXCAVATION. THE PROPOSED AREAS OF
EXCAVATION SHALL BE APPROVED BY EPA PRIOR TO EXCAVATION. THE LIMITS OF CONSTRUCTION SHALL BE
REVISED AS NECESSARY TO OBTAIN ACCESS AND TO COMPLETE EXCAVATION OF ALL WASTE AND
CONTAMINATED SOIL.

5. FOR PURPOSES OF BIDDING, REFER TO TABLE 4 ON SHEET G-02 FOR ESTIMATED WASTE AND CONTAMINATED
SOIL VOLUMES. THE DEPTH OF EXCAVATION SHALL BE BASED ON CONFIRMATION SAMPLING IN ACCORDANCE
WITH SECTION 02111.

6. EXCAVATED MINE WASTE AND CONTAMINATED SOIL SHALL BE PLACED WITHIN THE AREA 2 MINE WASTE
CONSOLIDATION AREA SHOWN ON THE CONTRACT DRAWING.

7. EXCAVATE MINE WASTE OUTSIDE OF MINE WASTE CONSOLIDATION AREA FOOTPRINT IN ACCORDANCE WITH
SECTION 02111. PLACE THE EXCAVATED WASTE WITHIN THE WASTE CONSOLIDATION AREA AS SHOWN ON THE
DRAWING AND IN ACCORDANCE WITH SECTION 02111. THE FINAL EXCAVATION SURFACE SHALL BE GRADED IN
ACCORDANCE WITH SECTION 02111.

8. REMOVE VISIBLE MINE WASTE ALONG THE SHORELINE OF THE POND. POND SHALL BE DEWATERED IN THE AREA OF
THE EXCAVATION SHOWN AND IN ACCORDANCE WITH THE CONTRACTOR'S APPROVED DEWATERING AND SEDIMENT
MANAGEMENT PLAN. REMOVE VISIBLE MINE WASTE AND 12 INCHES OF SEDIMENT FROM THE AREA SHOWN.

9. EXCAVATE THE CREEK CHANNEL, BANK TO BANK, TO A DEPTH OF 18 INCHES BELOW THE EXISTING CHANNEL
BOTTOM TO REMOVE MINE WASTE AND CONTAMINATED SOIL. REFER TO SHEETS CS-01 THROUGH CS-06 FOR TYPICAL
EXCAVATION DETAILS. PLACE THE EXCAVATED MATERIALS WITHIN THE MINE WASTE CONSOLIDATION AREA.
FOLLOWING EXCAVATION, PERFORM XRF SCREENING OF SOIL SAMPLES ALONG THE FLOW LINE AT 100 FOOT
INTERVALS. PROVIDE THE SCREENING DATA TO EPA FOR EVALUATION BEFORE PLACING RIPRAP AND OTHER SITE

NOTES:

1.	CONDUCT XRF ANALYSIS (FIELD XRF SCREENING) WITH PORTABLE
INSTRUMENT) LEFT AND RIGHT OF CREEK CHANNEL TO ESTABLISH LIMITS OF
EXCAVATION. SAMPLES FOR XRF ANALYSIS SHALL BE COLLECTED ALONG
TRANSECTS SPACED 50 FEET APART (E.G., STATIONS 0+50, 1+00, 1+50,...).

SOIL SAMPLES FOR ANALYSIS SHALL BE SPACED 10 FEET ON-CENTER ALONG
EACH TRANSECT. ALL SAMPLING AND ANALYSIS SHALL BE IN ACCORDANCE
WITH THE APPROVED QUALITY ASSURANCE PLAN REQUIRED BY THE
PERFORMANCE WORK STATEMENT. THE CONTRACTOR'S PROPOSED LIMITS
OF EXCAVATION SHALL BE APPROVED BY EPA PRIOR TO EXCAVATION.

2.	THE DEPTH OF EXCAVATION IS EXPECTED TO VARY FROM
APPROXIMATELY 6 TO 18 INCHES. THE FINAL DEPTH OF EXCAVATION SHALL
BE DETERMINED IN THE FIELD BASED ON CONFIRMATION SAMPLING
CONDUCTED BY THE CONTRACTOR.

3.	THE EXCAVATED AREAS LEFT AND RIGHT OF THE CREEK CHANNEL SHALL
BE BACKFILLED WITH CLEAN FILL (AS APPROVED BY EPA) AND TOPSOIL
(MINIMUM 6 INCHES) SPACED TO WITHIN 6 INCHES OF THE PRE-EXCAVATION
TOPOGRAPHY AND SLOPED TO ACHIEVE POSITIVE SURFACE DRAINAGE
TOWARDS THE CREEK CHANNEL.

4.	EXCAVATION ALONG THE BOTTOM OF THE CREEK CHANNEL IS REQUIRED
TO REMOVE MINE WASTE AND IMPACTED SEDIMENTS. FOR PURPOSES OF
BIDDING, THE CONTRACTOR SHALL ASSUME EXCAVATION OF UP TO 6 INCHES
OF MINE WASTE AND 12 INCHES OF IMPACTED SEDIMENT. THE DEPTH OF
EXCAVATION OF IMPACTED SEDIMENT SHALL BE MEASURED FROM THE
EXISTING CHANNEL BOTTOM. THE ACTUAL DEPTH OF EXCAVATION MAY BE
REDUCED IN THE FIELD BASED ON THE ABSENCE OF MINE WASTE AND/OR
THE PRESENCE OF BEDROCK AT SHALLOW DEPTHS. DURING EXCAVATION,
SOIL SAMPLES FOR XRF ANALYSIS SHALL BE COLLECTED AND ANALYZED BY
THE CONTRACTOR TO GUIDE DECISIONS BY EPA ON THE FINAL DEPTH OF
EXCAVATION. THE SOIL SAMPLES SHALL BE COLLECTED ALONG THE
CENTER-LINE OF THE CREEK CHANNEL AT 50 FOOT INTERVALS (STATIONS
0+50, 1+00, 1+50,...). WHEN EXCAVATION IS COMPLETED, THE CREEK
CHANNEL SHALL BE FLAT BOTTOMED AS SHOWN ON THE TYPICAL DETAIL.

5.	FOLLOWING EXCAVATION OF THE BOTTOM OF THE CHANNEL, THE
EXISTING SIDE SLOPES SHALL BE EXCAVATED TO REMOVE A MINIMUM OF 6
INCHES OF MATERIAL AND/OR TO ACHIEVE A MAXIMUM SLOPE OF 2.5
HORIZONTAL TO 1 VERTICAL. WHEN EXCAVATION OF THE SIDE SLOPES IS
COMPLETED, THE SIDE SLOPES SHALL BE SMOOTH AND UNIFORM IN
CROSS-SECTION TO RECEIVE RIPRAP, EROSION CONTROL MATERIALS, AND
SEED PLACEMENT WITHIN THE AREAS SHOWN ON THE DRAWINGS.
